Elevance Health Partners with NACHC to Integrate Food as Medicine into Primary Care

Elevance Health and the National Association of Community Health Centers (NACHC) announced on August 18, 2025, a partnership to integrate Food as Medicine (FAM) programming with primary care Community Health Centers (CHCs). This initiative expands Elevance Health’s existing regional Nourished Well program for Medicaid individuals. The program aims to connect Medicaid members at risk for diet-related chronic conditions to personalized nutrition and whole health supports, including medically tailored interventions, coaching, and referrals, as part of regular primary care interactions. Dr. Shantanu Agrawal, Chief Health Officer at Elevance Health, stated that this collaboration makes nutrition integral to care delivery. The Elevance Health Foundation will also support NACHC with a six-month FAM grant to assess existing programs, evaluate policies, and develop a scalable CHC FAM model. The long-term vision includes developing a Nutrition Center of Excellence to promote food-based clinical care nationally.
